To register to use the SmokeFreeSocietyEducation.org logo as an active link from your site, email us with the following information.

Notice - Please refrain from using SmokeFreeSocietyEducation.org logos (Logo link) until such time you have requested and received written permission from Smoke Free Society Corporation.

1. Name of person requesting this written permission

2. Company name

3. The nature of your business and its product and/or services

4. The reason for the link

5. The URL of your site where the SmokeFreeSocietyEducation.org logo will appear

6. Email address - must be valid to receive instructions and reply

7. Are you a new member (i. e. no prior use of the SmokeFreeSocietyEducation.org logo)

8. If you are a current member (i. e. prior use of the SmokeFreeSocietyEducation.org logo) please provide the proposed additional URL of your site where the SmokeFreeSocietyEducation.org logo will appear

9. By submitting this email request you indicate that you have read and fully agree with all the terms and conditions set forth in the Logo Link Agreement
